Van Memo of Understanding (28 kb)
This form must be received by Safe Kids Elkhart County and approved before any agency can borrow the SKBU Van. Please allow at least 4 weeks for processing.

Host a Car Seat Inspection (398 kb)
Please submit a completed request form at least four weeks prior to your event so that we may acquire supplies and arrange proper staffing.
